Patient Health Questionnaire – Depression (PHQ-9)
PHQ-9 is a 9-item scale designed to measure the level of severity of depression using the symptoms.
Depression Anxiety Stress Scales (DASS-21)
DASS-21 is a set of three self-report scales designed to measure the emotional states of depression, anxiety and stress.
Bipolar Recovery Questionnaire
This questionnaire serves to help you understand more about recovery in and experiences with the diagnosis of bipolar disorder.
Obsessive-Compulsive Inventory – Revised (OCI-R)
OCI-R is a 18-item scale that measures the severity of OCD symptoms and the likelihood of an OCD condition.
Social Avoidance and Distress Scale (SADS)
SADS is a self-report scale that measures level of social anxiety and what it may be associated to.
Generalised Anxiety Disorder Assessment (GAD-7)
GAD-7 is a 7-item measure designed for assessing levels of anxiety symptoms.
Outcome Rating Scale (ORS)
ORS is a brief tool to track therapeutic outcomes through the different areas of a client’s life.
Session Rating Scale (SRS)
SRS is a brief tool used to gather feedback from a counselling session.
Mindfulness-Based Self Efficacy Scale – Revised (MSES-R)
MSES-R is a 22-item scale designed to measure self-efficacy level in mindfulness skills.
Rosenberg Self-Esteem Scale (RSES)
RSES is a 10-item scale to measure level of self-esteem, where higher scores represent higher self-esteem.
Satisfaction with Life Scale (SWLS)
SWLS is a 5-item scale designed to measure level of satisfaction with life, where a higher score represents a higher satisfaction.
Pittsburgh Sleep Quality Index (PSQI)
PSQI is designed to measure sleep quality along 7 different components.
International Trauma Questionnaire (ITQ)
ITQ is a 18-item scale designed to indicate the likelihood of PTSD and CPTSD, as well as their respective severity levels.
PTSD Checklist (PCL-5)
PCL-5 is a 20-item scale that measures the severity of PTSD symptoms over 4 subscales that are consistent with the criteria in DSM-5.
Adult ADHD Self-Report Scale (ASRS)
ASRS is a 18-item scale that can be helpful in determining the subtypes and extent of ADHD symptoms that you may be struggling with.
Aggression Questionnaire
A 29-item questionnaire that measures aggression over the following subscales: Anger, Physical Aggression, Hostility, Verbal Aggression.
Self-Injurious Thoughts and Behaviours Interview (SITBI) – Short Form
This is a structured interview that asks about your thoughts and feelings towards self-injurious behaviours.
Eating Attitude Test (EAT-26)
EAT-26 is a 26-item screening measure to help you determine whether you might have an eating disorder that needs professional attention.
Alcohol Use Disorders Identification Test (AUDIT)
AUDIT is a 12-item scale designed to measure the likelihood of hazardous and harmful drinking.
Problem Gambling Severity Index (PGSI)
PGSI is a 27-item scale used to assess the severity of a potential gambling addiction.
